File and documentation review
Observation and/or direct interaction with the individual
Consultation with key stakeholders (e.g. family, carers, support workers, clinicians)
Using a holistic, clinical and functional lens, our team assesses how well the existing support model aligns with the person’s current needs, goals, and environment.
The final Service Model Planning Assessment Report includes:
