Multidisciplinary Case Consultation

Collaborative Planning for Complex Needs.
Integrated clinical insights to guide holistic, person-centred support.
Who is this for?
This service is for individuals with complex support needs—particularly where there are multiple challenges across health, behaviour, disability, trauma, or mental health. It is ideal for situations where existing support plans may be fragmented or not achieving desired outcomes, and a cohesive, team-based approach is required.
Multidisciplinary case consultations are often sought by:
Support coordinators or case managers
Health and disability services
Guardianship or care teams
Families or carers navigating complex systems
How does it work?
SAL Consulting brings together a tailored team of professionals, led by a Consulting Psychiatrist and/or Principal Psychologist, to provide a collaborative, clinical formulation of the individual’s needs, risks, strengths, and context.
Each case consultation includes:
A thorough referral and intake process to determine the most appropriate team
Review of available documentation and background
Clinical discussion informed by diverse expertise (e.g. psychology, psychiatry, occupational therapy, behaviour support)
A shared understanding of the individual’s experiences and goals
A clear, integrated plan for future support across services or systems
The outcome is a cohesive, evidence-informed direction for support, ensuring that recommendations are not only clinically sound but also practical, coordinated, and person-centred.
