Who are Individual Client Appraisal Reports (ICARe’s) for?
ICARe reports are designed for individuals with complex support needs, and are particularly helpful for support coordinators, guardians, case managers, clinicians, and services seeking a whole-of-life understanding of the person in order to plan more effectively.
They are commonly used when:
Coordinating services across multiple domains (e.g. health, housing, education)
A holistic view is needed to inform funding, transition, or support planning
Clarification is required around the individual’s strengths, needs, and future goals
How does it work?
An Individual Client Appraisal Report (ICARe) is a detailed, structured assessment that explores key lifestyle and support areas, including (but not limited to):
Accommodation and housing stability
Emotional and physical health
Education, employment, or vocational engagement
Daily living and functional skills
Social connection, support networks, and community access
The process includes:
Review of relevant documentation
Interviews with the individual and their support network
Direct observations where appropriate
Formulation of a clinical understanding of current functioning and context
The final report includes:
A clear summary of the individual’s situation across domains
Professional formulation and interpretation of key needs
Tailored recommendations to guide support and service planning
ICARe reports support teams to make informed, coordinated, and person-centred decisions that enhance outcomes and align with the person’s values and goals.
