Proactive Case Management
Target Group: Caseworkers and managers working with vulnerable families with complex needs: Targeting children and young people living in or out of the family system.
Therapeutic practice applications include: Neurodevelopment, Attachment, ecological supports and relational scripting.
Proactive case management assists a family and/or individuals when dysfunction significantly inhibits their ability to make change and movement toward a safer, healthier future. Proactive case management integrates a demonstrable therapeutic practice approach that works with client resistance and or reluctance to change: providing them experiences of authentic connection and shared goals, skills in co-regulation, develop an understanding of their strength areas that will lead to building resilience and learn how to take skills back into their ecology.
Participants learn how to use Perry’s Six-Core Strengths of Attachment, Self Regulation, Affiliation, Attunement, Tolerance and Respect – the foundation to healthy development in practice apply collaborative engagement strategies, targeted interventions and support, capacity building, and reflective practice to deliver integrated service delivery.
